PERSONAL DATA PROTECTION NOTICE OF THE SILSEAN UNIVERSITY OF MEDICINE IN KATOWICE

Pursuant to Article 13(1) and (2) of the General Data Protection Regulation (GDPR) of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation), hereinafter referred to as the GDPR, the Medical University of Silesia in Katowice (hereinafter referred to as the "Controller") provides the following information:

  3. Data is processed for the purpose and on the basis of:
    1. Article 6(1)(a) of the GDPR – The data subject has given their consent to the processing of their personal data for the purpose of processing the submitted form, solely for the purposes of providing the event presentation service.
  4. The recipients of personal data are the Medical University of Silesia in Katowice employees who are responsible for the content and who have been authorized to process personal data. Recipients may also be entities to which the University has entrusted the processing of personal data in connection with the provision of services such as technical assistance for software used to process orders, accounting, etc. Recipients may also be public authorities which have submitted a legally justified request for access to personal data.
  5. Data will be stored for a period determined on the basis of the Uniform List of Records in force at the Medical University of Silesia in Katowice, in particular for a period not shorter than the statute of limitations for financial claims arising from generally applicable laws.
  6. You have the right to request access to your personal data from the Medical University of Silesia in Katowice, to rectify, erase or restrict their processing, as well as to object to their processing, to the right to data portability, on the principles set out in the GDPR provisions.
  7. You have the right to lodge a complaint with the President of the Office for Personal Data Protection regarding the processing of your personal data.
  8. Providing personal data is voluntary, but necessary for the processing of the submitted form.
  9. In the case of the processing of the personal data provided, automated decision-making does not take place.
